The USA (short for the United States of America) is a rich country with a balanced climate. It shares a land border with Canada and Mexico, and has around 316 million people in-game, and can be very easy to infect because of the many airport links it has, linking with every country in the map with an airport.

IG Factbook: "Britain's American colonies broke with the mother country in 1776 and were recognized as the new nation of the United States of America following the Treaty of Paris in 1783. During the 19th and 20th centuries, 37 new states were added to the original 13 as the nation expanded across the North American continent and acquired a number of overseas possessions. The two most traumatic experiences in the nation's history were the Civil War (1861-65), in which a northern Union of states defeated a secessionist Confederacy of 11 southern slave states, and the Great Depression of the 1930s, an economic downturn during which about a quarter of the labor force lost its jobs. Recommendations

• As the USA is a rich country with a balanced climate, it is not required to evolve abilities such as Heat Resistance 1 and Cold Resistance 1 in order to excel in this country. Like other rich countries, the United States has a well-funded health care system, and thus it is inadvisable to start in the country, as it may take an incredibly long time to infect even a small population.  The only definite exception to this rule is if the player wants to win achievements for DarkWater when playing as the Necroa Virus. Nevertheless, it is imperative that that the player purchases Drug Resistance 1, as well as Drug Resistance 2 in Mega Brutal and Brutal difficulties.
• The United States may be a good choice to start the Simian Flu, it is the country with the largest ape population in the Americas, the reason for this is because it is a rich country with good sanitary conditions so human infections will not grow so fast, so it will not alert the world population so much, which will cause the disease will not be treated with high priority, this will give time to collect DNA points in addition to the apes being able to go more easily to other continents following various routes such as from Mexico to Colombia, Peru, Bolivia until reaching Brazil, in addition to routes that cross from South America to Africa, in which the apes arrive from West Africa until reaching Sudan or Saudi Arabia.

• There is a possibility of new elections in the USA after the president became ill due to the gambler's disease. Among the candidates is Spalin, a parody of the politician Sarah Palin, she will order the attack with nuclear bombs on China or Russia, because she believes that the governments of these nations have created the plague. For this to happen the player must evolve certain traits. These events are classified as Nuclear Retaliation and two different achievements can be obtained depending on which country Spalin attacks.
• If the player starts in the USA while playing the Necroa Virus, the game can say that a company named DarkWater created the Necroa Virus, a fictional American military company and research team, but the player must evolve Saliva 1 and Drug Resistance 1 within the first 70 days.
• If the player starts in this country in the Fake News scenario, the player can unlock the Independence Day achievement, which is a reference to a movie filmed in the United States.
• In the Ultimate Board Games scenario, the player can choose the United States to start the manufacturing process of his board game, after settling in this country, the final product will be less likely to have quality problems although it will be expensive. Such a manufacturing action will cost at least 30 funds.
• Between 2016 and 2020, several news headlines about the 45th President of the United States, Donald Trump were added to the game. These are:
  • "US President buys Condo in Greenland - claims victory" referring to the Trump administration's attempted purchase of the island from Denmark.
  • "Kim and Don livestream Fornite from Singapore", referring to the 2018 North Korea–United States Singapore Summit. Also, at the time this news was added, it was very popular to do a Fortnite livestream.
  • "Child tells President that 'injecting disinfectant is Not Good'" referencing the time Donald Trump in a 2020 speech said about injecting disinfectant as a measure to prevent diseases.
• In December 2019 the Armageddon expansion for Plague Inc. The Board Game was released, in which Tomorrow's World cards were introduced for some countries, among them, a card was added with the United States going through an isolationist period, meaning that this country no longer has access to any transport links such as airports or seaports.
• Despite Alaska being part of the United States, it is actually part of Canada in Plague Inc. Alaska is not part of the USA anymore, as the Canadians somehow acquired it via a treaty called the "Hay-Herbert Treaty." This was a joke put in by the game designers because an error in the game made it impossible for Alaska to be part of the USA.
  • Also, Hawaii does not appear on the map, because small islands are not included in the game.
• As of 2013, in Mutation 6, news headlines were added officially from the Centers for Disease Control and Prevention (CDC), a U.S. public health agency, in which there are several that talk about health data from this country, these include:
  • CDC: 38% of US adults don't walk >10mins per week
  • CDC: 20% of teen births are repeat births in the US
    • These have no effect on the gameplay and only serve to inform about some statistics and health information.
